Cause 1: it's still swelling (the most common)

Cause 3: visceral fat (the one no operation touches)
Fat packed around the organs behind the muscle wall pushes the whole abdomen outward — firm, round, drum-tight, and unreachable by any skin operation, because it's on the wrong side of the wall. Signature: was round before surgery too, doesn't pinch, doesn't change with position. The honest answer is metabolic — diet, exercise, medical weight-loss tools — and a revision sold for a visceral belly is money burned.
